Larry Combest Community Health & Wellness Center
The Patient Navigator Program, also known as the Transformacion Para Tu Salud Program is a chronic disease self-management program at the Larry Combest Community Health & Wellness Center, with a focus of serving the under-served minority populations in East Lubbock.
The Patient Navigator Program is based on the Transformation for Health conceptual framework for clinical services delivery, developed by the TTUHSC School of Nursing as a basis for practice, education, and research.
Utilizing Promotera's / certified community health workers as Patient Navigators, the program's goal is to coordinate comprehensive health services for patients in need of chronic disease care and management for the following health conditions:
- Diabetes
- Hypertension
- Congestive Heart Failure
- Asthma
With the Comorbidity:
- Depression
- Obesity
