Rotation: Gastroenterology Elective

Instructors: Drs. Tom Johnson and Amarillo Diagnostic Clinic gastroenterologists
Make up of Team:
- Clinical faculty attending will supervise both inpatient and outpatient exams
- 2 nd or 3 rd year resident
Days per Week:
- Monday to Friday
- No night calls (except one upper level ICU night call/month possible)
- No weekends
Usual Time of Attending Rounds:
- Usually in the morning, between 8:00 A.M. to noon
- May start at NWTH/BSA. Attending will inform you beforehand
Type of Patients Encountered:
Variety of patients with different GI disorders - in all age ranges 16 yrs and above
Educational goals/Competencies:
Patient Care:
- Outpatient evaluation of the patients with common GI complaints based on through history and physical examination and focused investigation
- Inpatient evaluation of patients with GI bleeding and abdominal pain
- Proficiency in performance flexible sigmoidoscopy and paracentesis
- Familiarity with EGD/colonoscopy, ERCP and liver biopsy
Medical Knowledge:
- Differential diagnosis and management of upper and lower GI bleed
- Evaluation and treatment of patient with acute and chronic liver disease
- Evaluation and treatment of patient with chronic hepatitis/alcoholic liver disease
- Evaluation and work up of patient with dysphagia/odynophagia and chronic aspiration
- Evaluation of the patients with esophageal disorders including GERD/motility and neoplasms
- Evaluation and management of patients with acid peptic disease and H.pylori related problems
- Evaluation and treatment of patient with a diarrhea and malabsorption
- Evaluation of patients with inflammatory bowel disease
- Evaluation and management of patient with colonic polyps and Malignancy
- Evaluation and management of patient with functional GI disorder, i.e. - irritable bowel syndrome
- Evaluation and management of patients with acute and chronic pancreatitis
- Indications and complications of common GI procedures - EGD/colonoscopy/ERCP/liver biopsy
- Evaluation of patient's nutritional status
Interpersonal/Communication Skills:
- Proper communication with patient and their family
- Communicate in chart and verbally with primary care as to your impression and plan of care after discussion with your GI attending
- Write daily notes on patients you are consulted on
- Explain in detail all the risks, benefits, and alternatives of procedures which are contemplated
Professionalism:
- Do consults in timely fashion
- Treat everybody with respect - including primary care team/patients/nurses endoscopy technicians
Practice Based Learning and Improvement:
- Learn from mistakes
- Learn from common and uncommon cases and do thorough literature search on cases which are not clear
- Discuss with your attending if any aspect of diagnosis or management is not clear to you

Method of Evaluation:
Will be done by the GI attending who has most exposure to you. It will be based on your thoroughness and timeliness of consult, knowledge of GI disorders, willingness and commitment to learning, and your procedural proficiency. Further input may be obtained from patient/family members and nurses.
