Updated Confidential Communication Request & Identity Theft Protection Questions Form

Please be aware that there is an “updated” version of the Confidential Communication Request and Identity Theft Protection Questions form. You can find the latest and greatest version at the related links below. Please use this form from this point forward. If you have already obtained the previous “new” form (dated February, 2012/March 01, 2012) then you do not need the patient to complete it again. The form with a date of Feb/March, 2012 or April 2, 2012 should be on file with every current patient.
