Please fill out a Telephone Work Order Request Form Please submit to Communication Services in Medical Center Bldg., Rm D-26 (basement), send via campus mail, or fax to 545-9726. NOTE: If requesting an add, move or change to your existing phone, please remember that it takes six to seven days time process due to interfacing with Southwestern Bell and Communication Services-Lubbock, so please plan ahead. Work Order Requests must be approved and signed by your department administrator or authorized person.
All requests for new voice mail or changes in existing service should go through your department's Administrator signature and approval.
