Inclusion of oral cavity and oral hygiene screening of people with Type-2 Diabetes with referral for oral health treatment (minimum prophylaxis services) with concurrent diabetes self-management education (DSME) and Chronic Disease self-management education (CDSME) with tracking of glucose/HbA1c pre-, inter-, and post treatment.
