In 1999, then-Texas Tech Chancellor John T. Montford envisioned a full fledged four-year medical school in El Paso, primarily due to the physician shortage on the border. The El Paso community showed support for that vision and joined forces to gain legislative support to convince the Texas Medical Association and the Texas Higher Education Coordinating Board that it was time to change their formal position opposing any new medical schools being built in Texas.

In 2001, philanthropists J.O. and Marlene Stewart donated 10.2 acres of land, while the Paso del Norte Foundation approved a $1.25 million dollar scholarship grant program for local students with the approval of the four-year medical school.
