Physical Exam Form
A physical exam form is included. This needs to be filled out by the faculty or a resident after you are observed doing a complete history and physical. Again, this is to ensure you have acquired the basic skills of interviewing and examining the female patient. The history and physical exam form need to be turned in at the end of the rotation before a grade will be assigned.
