Welcome to the Texas Tech University Health Sciences Center Gayle Greve Hunt School of Nursing 2nd degree BSN Program, Spring 2012 Cohort! You are required to complete the following on-line modules BEFORE your Orientation: HIPPA Training, EEO-Sexual Harassment Training, and CWO.You must submit your certificate of completion for each individual module to: Gretchen Ruiz via e-mail, fax, or mail to the address below no later than December 14, 2011. Please follow the directions below to do so.

STEP 2: Complete Online Training Modules

- "Criminal Background Check” ~ this link will go live on 12/14/11. Please return at that time to complete and submit payment for this Mandatory CBC. In the mean time, you should complete and sign the attached CBC Release Form, and return to Gretchen Ruiz via e-mail, fax, or mail to the above address.

STEP 3: Immunizations-Please complete in the following Sequence!
- Tuberculosis skin Test- Must be completed unless you can provide documentation of TST completed within the last 3 months!
- Have Titers (lgG) drawn, and submit LAB Reports for the following:
- Hep B
- Varicella
- Rubella
- Rubeola
- After obtaining TST Results, Take both the TDAP Vaccine (MUST include Tetanus, Diphtheria, AND Pertusis) AND Meningococcal Vaccines
- Submit Documentation for the above vaccinations, lab reports, and test. Contact Gretchen w/any questions or concerns!
