Welcome to the Faculty Nominations website. All nominations will be received electronically. Please read OP 10.25 prior to submission. The award rubrics are provided to inform candidates of evaluation criteria.

Faculty Awards - Academic Year 2013
Instructions for Applications

The Faculty Awards Committee requests the following instructions be followed for all applications submitted:

  • The number of pages in the application packet is a maximum of eleven (11). (See Exception).

  • Many of the basic scientists may wish to submit a two-page NIH biosketch, however, clinicians may wish to submit an abbreviated CV (again only two pages). In either the NIH biosketch or abbreviated CV, the reviewer wishes to see (based on the award being nominated for) relevant nominee information such as publications, awards, etc.
    • Exception: The number of pages on the application packet for the President’s Award for Excellent in Interprofessional Teamwork is thirteen (13) (if two individuals are on the team).

    • For each additional team member, a two page NIH bio-sketch or abbreviated CV can be added in addition to the thirteen pages.

  • The eleven (see Exception) pages should address each of the criteria as outlined in the rubrics.

  • Order of completion/submission:
    • Award Name field
    • Award Candidate Name field
    • School field
    • Telephone Number
    • E-mail Address
    • Award Candidate Rank
    • NIH Biosketch or Abbreviated CV (2 page maximum.)
    • Summary statement of accomplishments by criteria for specific award ( 1 page maximum)
    • Discuss each of the rubric blocks for a specific award in the order as mentioned in the rubric page. (8 page maximum. See Exception)

      EXAMPLE: President’s Excellence in Community Engagement Assessment: start with Years Of Service, Time Commitment, Local and National Awards, Role Modeling, and finally, Impact On Community.

  • The document must have a font size no smaller than 12 pt, margins no smaller than 0.75” and preferably be double-spaced.

  • PDF's are the preferred file format.

  • Annual Faculty Review documents will NOT be accepted as part of the application packet.

The committee will STOP READING after page 11!!!
