For enrollment of your dependents, you will be required to provide copies of documents to the insurance representatives at Orientation on July 2, 2012. The Health & Vision presentation provides a list of eligibility requirements and documents you need to bring with you on July 2, 2012. (see slide 3, #7, and slides 16-18)
