Templates/Deliverables

ITPMO classifies projects into 4 levels: Major, 1, 2, or 3. Below are the required templates for each level. If your project has already been classified, click here to fill out the appropriate templates. NOTE: Functionality will diminish with Firefox, we highly encourage using Internet Explorer to fill out the templates.

A Change Request must be submitted to the ITPMO for any proposed project changes.

INITIATION

Deliverable Description Required for Level(s)
Project Request This form serves as project initiation for the ITPMO. Determinations will be made with regard to the project legitimacy Major, 1, 2, 3
Outline Provides comprehensive information used to establish level Major, 1, 2, 3
Initial Project Estimate Includes a Rough Order of Magnitude (ROM) budget and schedule Major, 1, 2, 3
Business Case Justifies the project; provides information to make an informed decision to proceed further Major, 1, 2, 3
Project Charter Will formally authorize work to begin on a project. The intent is that an agreement among stakeholders is established before significant resources are committed and expenses incurred. Major, 1, 2, 3
Business Justification Review Gate Will help determine whether a project is ready to proceed to the next phase Major

PLANNING

Deliverable Description Required for Level(s)
Statewide Impact Analysis Measures collaboration and business processes across state agencies Major
Project Plan Defines activities and resources needed to deliver the project's product and/or service Major, 1, 2, 3
Acceptance to Deploy Provides formal product and/or service acceptance before a product and/or service becomes operational Major, 1
Acquisition Plan Will help plan and ultimately procure goods and/or services through management of solicitations and contracts Major, 1 if applicable
Contract Amendment and Change Order Approval Ensure project changes that impact a technology contract are analyzed and justified before funds can be expended for those changes Major, 1 if applicable
Contract Advisory Team Review and Delegation Application (CATRAD) Any agency major contract solicitation over $1 million in value must be submitted to the CAT team for review. Any agency contract solicitation (IFB or RFP) for the purchase of services over $100,000 per year must be submitted to TPASS (CATRAD) as a request for delegated purchase authority. Major, 1 if applicable
Technology Addendum Offers state agency contract managers recommendations on improving existing contract management processes and practices Major
Project Planning Review Gate Will help determine whether a project is ready to proceed to the next phase Major
Solicitation & Contracting Review Gate Will help determine whether a project is ready to proceed to the next phase Major if applicable
