Investigator Agreement - required to be signed by the Principal Investigator and Department Chair for initial review submissions to the IRB for all initial IRB submissions in iRIS.

HIPAA - HIPAA (Health Insurance Portability and Accountability Act) rules which went into effect April 14, 2003 are applicable to research projects carried out at TTUHSC in which protected health information (PHI) is obtained. In order to be in compliance with HIPAA regulations, research proposals which include PHI must contain either an "Authorization to Use and Disclose Protected Health Information for Research" or a "Request to use and disclose PHI without authorization for research purposes." These forms are available in IRIS. Additional information regarding the use of protected health information for research purposes can be found in the Investigator's Guideand at www.ttuhsc.edu/hipaa.
