Physician Transcripts of Credit
The Office of Continuing Medical Education maintains transcripts of credit earned for physicians (MD's and DO's only) for activities certified by Texas Tech University Health Sciences Center. Transcripts are $10 each. TTUHSC departments may use a departmental transfer to pay for faculty transcripts by sending the account number to charge along with the request. Individuals may pay via personal check, credit card or cash.
To request a transcript, send the following information in writing:
- Your full name
- last four digits of your Social Security #
- address to which the transcript should be sent, and
- period of time for which you are requesting a transcript
