Verification of Residency
All residency and fellowship verifications must be in writing. Fax or mail to Office of GME. A signed release must be included. Make sure to include whole name (including maiden name if applicable), last four numbers of social security number, and program.
Please note: The GME Office will verify dates of service only. For a detailed evaluation you may either send directly to the department or send to Office of GME and we will forward to the appropriate department.
