Larry Combest Community Health & Wellness Center
The Patient Navigator Program, also known as the Transformacion Para Salud Program is a chronic disease self-management program implemented at the Larry Combest Community Health & Wellness Center targeted towards under-served minority populations in East Lubbock.
The Patient Navigator Program is based on the Transformation for Health conceptual framework for clinical services delivery, which was developed in the TTUHSC Anita Thigpen Perry School of Nursing as a basis for practice, education and research.
Using a group of Promotera with Community Health Worker Certification as Patient Navigators, the program aims to coordinate comprehensive health services for patients in need of chronic disease care and management for the following health conditions:
- Diabetes
- Hypertension
- Congestive Heart Failure
- Asthma
- Depression
- Obesity
