Application Information

Interested applicants should forward their application materials no later than:

The application should include:

  1. Completed application form (click to download)
  2. Letter of interest (describe your interest in the residency program and your professional goals and objectives)
  3. Curriculum vitae
  4. Official transcripts from all pharmacy education
  5. Three (3) letters of recommendation (Letters of recommendation should be signed and may either be emailed; faxed directly to (214) 372-5020; or mailed to the address below.)

*Only 1 application packet is required regardless of the number of programs to which you are applying. When applying to multiple programs, letters of recommendation should be addressed to Wendi Monroe at the address noted above and will be distributed to individual program directors after the application is complete.

**If you are applying ONLY to the VA North Texas Healthcare System, the application packet can be sent directly to:
