Confidentiality Statement

A medical record is created when you receive treatment from a medical professional at Student Health Services. Each student's medical record is the physical property of Student Health Services and must remain in the custody of SHS staff at all times. Consistent care is exercised by all SHS staff to protect the privacy and confidentiality of patient health information (PHI). Medical information is not released by any member of the SHS staff without a student's consent unless authorized by law.

Information about computers requesting pages on this site, including IP addresses may be retained for statistical reporting. Personal information you enter into forms on this site is recorded in databases and will be used only for the purpose stated on the form and any provided description of the form.

Medical Records

Your medical records are the physical property of Student Health Services. The information contained in the medical record is confidential and will not be released without your permission unless otherwise authorized by law. Patients can submit an Authorization for Release of Patient Information to request access or copies of their medica records. Most requests for copies can be accommodated within 7-10 business days. Please contact us at least 24 hours in advance to make arrangements to view records or request copies for pickup. To help protect your privacy we require that present identification prior to viewing records or picking up copies from our office.
