Doctor of Pharmacy
- COMPLETE the School of Pharmacy Online Application - Apply Online
International Students must also complete a supplemental application - Submit Required Documents:
- Official Transcript(s) from each regionally accredited U.S. College or University attended
(A course-by-course evaluation is required on any courses from outside the U.S. only if they are used for prerequisites. If you do not use any courses from a foreign transcript for prerequisites, then an evaluation is not required.) - Pharmacy Experience Essay (as part of the online application)
- Non-Refundable Application Fee of $100.00 (please be sure your social security number is on your check)
- Three letters of reference on Texas Tech School of Pharmacy forms (can be downloaded from the online application)
- Pharmacy College Admissions Test Scores (official scores must be received from the Psychological Corporation by the deadline date)
- Official Transcript(s) from each regionally accredited U.S. College or University attended
