Multidisciplinary Assessment
The Integrated Geriatrics Practice takes an interdisciplinary approach when assessing complicated patients. Please consider sending elderly patients with complex medical conditions to the Integrated Geriatrics Practice, conducted on Friday mornings by the departments of Family Medicine, Internal Medicine and Neuropsychiatry.
Practice Highlights
- Patient evaluation by multidisciplinary team including a geriatrician, neurophsychiatrist, primary care physician, pharmacist, social worker and nurse care manager.
Access to nutritionists, behavioral scientists, nurses, and other health care professionals as warranted.- Treatment and therapy recommendation sent to referring physician.
- Referring physicians welcome to participate in the multidisciplinary visit.
- The Integrated Geriatrics Practice does not supplant the patient's primary care provider; all patients are sent back to the PCP.
- Most insurance plans accepted.
