Signs & Name Badges
The Facilities' office takes requests for almost all signage for the Texas Tech University Health Sciences Center at the Permian Basin. Medical Center Hospital provides a picture identification name badge for faculty, staff and students. Contact Bill Hefner, HSC Security, at 432-335-5279 to schedule a date and time to have this done.
Signs
Various signs are ordered through the Facilities' office: Room signs; insert signs, usually with name and title; inside directional and instructional signs; off campus clinic signs, etc. Submit the Sign & Name Badge Form . Most signs will require an NCRF - New Construction Request Form. Please call the Facilities' office to determine if a New Construction Request Form (NCRF) is needed. The Facilities' office will provide this form, if you do not have one immediately available. These forms can be ordered from Copy/Mail at TTUHSC in Lubbock, 806-743-2094 at a cost of $.95 each.
UPON SEPARATION, BADGES ARE REQUIRED TO BE TURNED IN TO THE HUMAN RESOURCES OFFICE.
