COVID-19 Sick Attestation Form: 

TTUHSC team members (faculty, staff, and students) who are exhibiting symptoms and/or have a temperature greater than 100 degrees Fahrenheit, or who may have been in close contact with someone known to have tested positive for COVID-19 are required to submit an online screen form to the TTUHSC Office of Institutional Health.


The Texas Tech University Health Science Center Office of Institutional Health strives to provide students and faculty preventive solutions to transmissible disease known to be of risk in health care.

Health care exposures can be minimized through the use of immunizations, annual Flu vaccine, and proactive exposure management.

It is the priority of our department to limit these risks and protect the health of students, faculty, employees and volunteers.

During this difficult time, please know that we understand it may be difficult to get your immunization/titer requirements complete. Please send in what you currently have including your pediatric shot records and any other records you may have.
