Define and Track Your Personal Wellness Goals

As a HealthSelectSM participant, you have access to the Well onTarget® Health Assessment. This is one of many resources that can help you improve your health and wellness.

screenshot of health assessmentThe Health Assessment takes about 15 minutes to complete. It asks several confidential questions about your lifestyle and health habits to provide an overall assessment of your current health. It identifies what you are already doing well—and shows where there is room for improvement. The Health Assessment adapts questions and suggestions based on your input and provides evidence-based action steps to assist in changing behaviors.


If you are taking the Health Risk Assessment as part of the Employee Wellness Leave program, you will need to have completed a physical examination with a health care provider prior to completing the assessment. That way, you will have the health information you need, like lab work results, to answer assessment questions accurately.

After you complete your assessment, your Well onTarget portal will be tailored to your specific needs. Your dashboard will display relevant programs that may help you reach your goals.

You will also receive a Personal Wellness Report, which includes:
  • information about health risks,
  • wellness recommendations,
  • preventive care recommendations,
  • suggestions for next steps in your health and wellness journey and
  • recommendations for other health and wellness resources.

You can also compare your results with previous reports if you have taken the Health Assessment before. 

Once you complete your Health Assessment, you will receive a certificate (it will not contain any personal health information). This certificate can be printed or downloaded to be provided as supporting documentation when submitting the Wellness Leave Certification form. 

To access your certificate, go to the left-hand menu on the top of the Well onTarget website, and click on “Certificates.” All prior certificates are listed here as well.
