Child and Adolescent Psychiatry Track and Fellowship Lubbock

Our philosophy at TTUHSC Department of Psychiatry is to provide you with a fundamental knowledge base and to equip you with the skills and tools you need to pursue a journey of lifelong learning. We facilitate an adult learning model with a mix of lecture-based material and group interactive learning through seminars and problem-based learning cases. This model requires the learner to engage in reading and investigating prior to the designated didactic time in addition to preparing questions for discussion.

A child and adolescent psychiatrist is a unique and valuable resource, and it is important that you become proficient in both diagnosis and treatment planning in straightforward and complex presentations. To facilitate your education, we will engage a multidisciplinary team of experts.

Tuesday afternoon didactics are designed to incorporate longitudinal seminar learning, where your knowledge will build over time, disease-based lectures, and special topics. You will be exposed to each disease-based lecture twice over the two-year fellowship curriculum. Some special topics are reviewed twice, and some just once over the course of the two-year fellowship learning period.

Supervision

Supervision Goals: Personal growth to support professional development. A space to discuss clinical, system, and interpersonal challenges that affect health and happiness in medicine. Additional clinical supervision is provided during clinical activities.

• Supervision will run from August through May
• Friday afternoons from 3 to 5 PM are reserved for supervision time. Your supervisor may choose a different time during the week as long as it works with your schedule.
• FY1 receives 1 hour of supervision weekly from a psychology supervisor and 1 hour of supervision weekly from a psychiatry supervisor. Supervisor changes at the 6-month mark.
• FY2 receives 1 hour of supervision weekly from a psychology supervisor and 1 hour of supervision weekly from a psychiatry supervisor. Supervisor changes at the 6-month mark.

Consult Liaison

Experience: Consult/Liaison Child and Adolescent Psychiatry Rotation will provide Residents/fellows an opportunity to learn how to best evaluate child and adolescent patients in the general hospital setting and how to best triage these patients for further care, including inpatient psychiatric care, day treatment, outpatient follow-up, and referral to other community resources. Resident/fellows will gain experience in assessment and treatment of emergency presentations such as suicidal ideation, homicidal ideation, delirium, serotonin syndrome, neuroleptic malignant syndrome, and prescribed and illicit substance overdose. In addition, residents/fellows will engage in treatment teams regarding psychiatric sequelae of acute and chronic illness. Residents/fellows are expected to work with the referring physicians and ward teams, and to collaborate with them in the delivery of comprehensive patient care.

Pediatric Neurology

The Pediatric Neurology Rotation includes an inpatient consultation and outpatient clinical experience involving the workup of new patients referred by community providers, and the follow-up of known patients. There will be emphasis on prevention, counseling, and screening for neurological pathology, normal vs. abnormal neurological development, undifferentiated neurological signs, and common presenting neurological conditions and their treatment.

Adolescent Medicine

The Adolescent Medicine Rotation is mainly an outpatient clinical experience involving the workup of new patients referred by community providers and the follow-up of known patients. There will be emphasis on eating disorders, menstrual disorders, mental health, family planning assistance, sports medicine, and issues related to gender identity and sexual orientation. Limited inpatient clinical experience involving medical stabilization of patients with anorexia nervosa and consultations on patients with abnormal vaginal bleeding.

Child Protection Pediatrics

The Child Protection Pediatrics program offers forensic evaluation, preventive education, multidisciplinary reviews, and expert medical testimony for children who may have been abused or neglected. Our services strengthen the foundation of subsequent legal and protective action and facilitate necessary treatment and follow-up for children who are victims of physical, sexual, emotional, or medical abuse.

Developmental & Behavioral Pediatrics

The Developmental and Behavioral Pediatrics Rotation is mainly an outpatient clinical experience involving the workup of new patients referred by community providers, and the follow-up of known patients. There will be emphasis on prevention of developmental and behavioral pathology, distinguishing normal from abnormal development and behavior, anticipatory guidance for appropriate development, undifferentiated signs of developmental pathology, and commonly referred developmental diagnoses. Two days will be scheduled at the Burkhart Center for Autism Education and Research to gain more experience in diagnosis and therapeutic management of autism spectrum disorders, including observation of Applied Behavioral Analysis. Prior to attending this experience, the resident/fellow will be expected to complete the Centers for Disease Control module on Autism Case Training web-based continuing education course. Time will be provided on this rotation to complete this module.

Forensic Psychiatry

Fellows engage in the justice system for the evaluation and medication management services, to children aged 18/under who are incarcerated at the Lubbock County Juvenile Justice Center (LCJJC)

Child Psychiatry Access Network, Urgent Assessment Clinic

Clinician-to-clinician program offering real-time access to a multidisciplinary network of mental health experts for Pediatricians and child primary care providers.

Medication Management Continuity Clinic

Provide diagnostic evaluation and medication management services in an outpatient clinic to children aged 18/under; trainee-led clinic with CAP psychiatric provider supervision.

Therapy Continuity Clinic

Provide psychotherapy services to outpatients aged 18/under (Psychodynamic, CBT, DBT, and ACT); trainee-led therapy with supervision by psychotherapist.

Day Treatment 

Hospital-based day treatment for children who are 10-17 years old. The therapist-led program provides children with intensive therapeutic treatment five days a week, partial hospitalization, or 2-3 days a week intensive outpatient.

Texas Child Health Access Through Telemedicine (TCHATT)

TCHATT provides telemedicine or telehealth programs to children and adolescents, grades Pre-K through 12th, via referrals from regional school district personnel. TCHATT assists in directing counseling and telepsychiatry to students, linking families with resources, and providing education to families and school districts.

Medication Management Continuity Clinic

Provide psychiatric evaluation and medication management services in an outpatient clinic to children aged 18/under; trainee-led clinic with supervision by Child and Adolescent Psychiatrist.

Lubbock Independent School District (LISD)

Provides the opportunity to observe children in their classroom settings to gain a deeper understanding of behavioral and learning challenges. Following outpatient clinic evaluation, fellows collaborate closely with the child’s guardians and school personnel to develop and guide appropriate support and educational accommodations.
