Comprehensive Memory Clinic Services
Our Memory Clinic specializes in evaluating and managing conditions that affect memory, thinking, behavior, and other cognitive abilities. We take a patient-centered approach that includes family members and care partners as a vital part of the care team. Our goal is to work with you to better understand the changes you may be experiencing, identify the underlying cause, and connect you and your loved ones with guidance, supportive services, and helpful resources every step of the way. Our Memory Clinic is unique as we have a comprehensive team made up of a neurologist, geriatric psychiatrist, neuropsychologist, speech language pathologist, clinical social worker, and numerous supportive staff members.
Garrison Institute on Aging Services
The Garrison Institute on Aging (GIA) Memory Clinic in Lubbock offers a range of expert-led support services, including Creative Minds, Connected Caregivers, Grieving Together, and the GUIDE Program, providing meaningful activities for individuals with dementia and compassionate respite and guidance for their caregivers. The GIA and the GRILL Lab introduces the Centers for Medicare & Medicaid Services (CMS) GUIDE Program. See wovenly for more details. This program provides free services to the person with dementia and their family member.
Beneficiaries receive timely referrals to specialists to address other health issues, such as diabetes, and the care navigators coordinate care with the specialist.
CMS GUIDE Eligibility
Eligibility for CMS GUIDE Service is determined by:
- Dementia Diagnosis (Can be determined at first visit)
- Enrolled in Medicare A & B
- Not Residing in Long Term Nursing Home
- Not Enrolled in Medicare Hospice
- Not Enrolled in the Program of All-inclusive Care for the Elderly (PACE)

CMS GUIDE Services
- Comprehensive Assessment
Beneficiaries and caregivers receive separate assessments to identify their needs and a home visit to assess the beneficiary's safety. - Care Plan
Beneficiaries receive care plans that address their goals, preferences, and needs, which helps them feel certain about next steps. - 24/7 Access
Beneficiaries and caregivers can call a member of their care team or a third-party representative using a 24/7 helpline. - Ongoing Monitoring & Support
Care navigators provide long-term help to beneficiaries and caregivers so they can revisit their goals and needs at any time and are not left alone in the process. - Referral & Support Coordination
Beneficiaires' care navigator connects them and their caregivers to community-based services and supports, such as home delivered meals and transportation. - Caregiver Support
Caregivers take educational classes and beneficiaries receive respite services, which helps relieve the burden of caregiving duties. - Medication Management
Clinician reviews and reconciles medication as needed; care navigators provide tips for beneficiaries to maintain the correct medication schedule. - Care Coordination & Transition
Beneficiaries receive timely referrals to specialists to address other health issues, such as diabetes, and the care navigators coordinate care with the specialist.
The TTUHSC Garrison Institute on Aging Alzheimer's Disease Awareness Month (an annual campaign occurring in November) serves as a platform to confront these daunting statistics, rallying communities, healthcare professionals, caregivers, and individuals alike. By disseminating accurate information, dispelling myths, offering support, and fostering research, this campaign strives to inspire collective action in the fight against Alzheimer's disease and to ultimately improve the quality of life for those affected by this relentless condition.
A Free Recreational Activity Group for Persons with Dementia and Respite for 6 hours a week for the Caregiver. This is at the GIA.
This is a support group that meets monthly for 2 hours for caregivers of persons with dementia. We meet the 3rd Tuesday from 10 am to 12 pm at the GIA.
This is a support group that meets monthly for 2 hours for family members who have lost a person to dementia. We meet the 1st Tuesday from 10 am to 12 pm at the GIA
